Social Support Groups provide eligible older Italians with an opportunity to socialise with their peers in a supportive, culturally appropriate, centre-based environment. Our social planned activity groups are offered in the North East, North, West and East regions of Melbourne.

The Social Support Groups aim to assist those who are physically frail, have dementia or other types of cognitive impairment or find it difficult to participate in general social groups. In addition to the socialising aspect of the program, these groups assist individuals in maintaining and enhancing their independence and connection to the community. Activities reflect partecipants' interests and focus on maintaining daily living skills as well as meeting their physical, emotional and social support needs. Partecipations at these Social Support Groups gives family carers much needed respite. Group members are supported by qualified Italian speaking staff.

Enquiries and referrals

Consumers wishing to attend the Social Support Groups require an assessment by the Regional Assessment Service. This can be arranged by contacting My Aged Care on 1800 200 422 or visit My Aged Care website for a referral code to access the service provided by CO.AS.IT.. Once the referral is made, the program Coordinator will organise a time for you to visit one of the Centres and together will develop a Care Plan which takes into account your individual interests, preferences and care needs.

The assessment will involve the client, their carer or other people in their life, including family members, a case manager, etc.

Transport

Transport can be provided via taxi using multi-purpose taxi cards (if available).

Fee

A nominal daily attendance fee applies.

The CO.AS.IT. advantage

CO.AS.IT. provides a culturally specific service geared towards addressing the needs of members of the Italian-Australian community. Our focus is on Italian-specific tradition, culture and way of doing things. The individual care plan is guided by the needs and wishes of the personattending the Centre. All Social Support Groups' staff speak Italian and understand the culture.
