The Access and Support Program is a free and person-centred service offering short-term, individual support to eligible people of Italian background and their carers to understand, access and negotiate the range of services available through the Commonwealth Home Support Program.
As part of this service, CO.AS.IT.:
- provides information on Commonwealth Home Support Program and community support services available to older Italians living in the inner North and inner West Metropolitan Regions
- offers assistance in making contact with the My Aged Care contact centre and provides support throughout the registration and eligibility screening process
- offers assistance in dealing with the Regional Assessment Service and Aged Care Assessment Team during the assessment process
- works in partnership with other agencies involved and people who support you such as family members, friends, your community, your doctor and other health professionals
The CO.AS.IT. advantage
CO.AS.IT. provides a culturally specific service geared towards addressing the needs of the Italian-Australian community. We understand Italian-specific tradition, culture and way of doing things. The individual care plan is guided by the needs and wishes of the person needing care and, if applicable, their primary carers. All program coordinators have knowledge of the Italian language (and dialects) in order to best facilitate communication and understanding.
